Dixie, GA - Jack Walker Tate, 16 months, of Dixie, Georgia passed peacefully on March 18, 2026 at Wolfson Children's Hospital in Jacksonville, FL. He was born on October 20, 2024 in Thomasville and was the beloved son of Seth and Heather Tate.

He was a precious baby boy that left a big impact on all who met him with his big blue eyes and sporty attitude. Due to his Menkes Disease he endured many challenges in his short life but managed to fight courageously, showing everyone what he was capable of at his young age. His parents were truly honored to care for him, treasuring every bit of snuggles until the end.

The family extends abundant gratitude to Dr. Amy Cooper and staff at the Pediatric Center, Tiffany and Dr. Tom Truman, Dr. K. Golec and the staff at the TMH Children's Center, the staff at Wolfson Children's Hospital, Dr. Kristin Parsley, and Dr. Holly Crowley for providing exceptional care for Jack and his parents.

He is survived by his siblings Madilyn Tate and Ella Tate of Dixie, his maternal grandmother Christi Leik of Boston, GA, paternal grandmother Rhonda Tate of Pelham, GA, aunts and uncles Caley Armstrong, Ashton Barrett, Logan Leik, and Kristy Tyler, and a number of cousins. He was preceded in death by his brother Maddox Tate and paternal grandfather Jack Waters.

In lieu of flowers, donations can be made to the Tallahassee Memorial Foundation to benefit the Children's Center.

A Celebration of Life will be held at Victory Fellowship Church at 2:00 pm on Tuesday - March 24, 2026 with Pastor Ann Nunnally officiating. A Visitation will be held on Monday - March 23rd from 5:00 pm - 7:00 pm at Whiddon Shiver Funeral Home chapel.
